Health Wealth Fund One – Series 2 (Medical NNN Portfolio)

Health Wealth Capital acquires and operates single-tenant medical real estate across the United States: dental, veterinary, and specialty medical buildings leased to healthcare providers on long-term triple-net (NNN) leases. Founded in 2017 by A.J. Peak, who built Peak Dental Services into a ~$100M, 50-location dental service organization, Health Wealth Capital underwrites the tenant's practice as rigorously as the building itself.

Today the portfolio spans 90+ buildings owned or under contract across 26 states, with $11M+ in annual net operating income and zero vacancies or delinquencies since inception. Roughly three-quarters of NOI comes from Tier 1 tenants backed by institutional private equity sponsors such as Warburg Pincus, Linden Capital, and Morgan Stanley Private Credit.

Accredited investors participate through series funds targeting 7-8% cash-on-cash distributions paid monthly, an 8% preferred return, a 16-20% base-case IRR, and a 2-3x equity multiple over a five-year hold, with an aggregation-to-REIT exit strategy and meaningful first-year depreciation. Health Wealth Fund One Series 3 is currently open.
